Position Overview


POSITION OVERVIEW:
Provide intensive case management to clients assigned; ensure accurate tracking and timely submission of milestones and outcomes for all ATI clients in the program with the goal of achieving all contract objectives.
 
KEY ESSENTIAL FUNCTIONS:
• Provides direct case management services to 15-20 clients.
• Provide oversight, case management to remaining PMI/ATI cases not directly assigned
• Conduct psychosocial assessment, if required.
• Prepare treatment/service plan.
• Conduct group services that relate to PMI/ATI Goals: HIV, Financial Planning & Family Reunification.
• Coordinate on-site and off-site referrals.
• Maintain communications with Parole officer and/or court personnel, attorneys, etc., if required.
• Write and submit monthly court feedback.
• Assign and coordinator client/family to the appropriate services.
• Attend interdisciplinary team meetings, case conferences.
• Monitor clients’ progress and ensure that court mandates are met.
• Submit accurate and timely monthly tracking sheets as required by contract.
• Track client milestones and outcomes to ensure contract objectives are met.
• Collaborate with all appropriate staff to ensure targets are met for contract compliance.
• Attend and participate in monthly ATI meetings.
• Perform other tasks or duties as required.


Qualifications



EDUCATIONAL / TRAINING REQUIRED:
• CASAC, CASAC-T or related degree.
 
EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:
• Bi-lingual (Spanish/English)/bi-cultural.
• Direct experience in providing clients services to ATI population
• Knowledge of the complex interaction of multiple factors of substance abuse and mental health preferred with sensitivity to cultural and gender specific issues with both individual and families.
• Knowledge of Criminal Justice and related system, policies and procedures.

COMPUTER SKILLS REQUIRED:
• Basic computer skills required for tracking contractual targets.
