Position Overview

Position Overview:
Under the supervision of the Senior Case Manager, the Case Manager provides case management to individuals and families in need of homeless prevention services. He/she will oversee case management for program participants and their families which include arranging, coordinating, monitoring and preparing and submitting Financial Assistance Requests on behalf of program participants. Advocate on behalf of program participants to ensure they receive work support services such as rental assistance, food stamps, Medicaid, subsidized childcare, etc. Promote linkage with other service providers that will help participants obtain and maintain housing stability.

Qualifications


Requirements:
  • BS in related field and one year of qualifying experience OR
  • AA degree in related field and two years of qualifying experience
  • Bilingual English / Spanish preferred
  • Excellent case management skills
  • Position requires energy and interest for home visits, case finding and outreach
  • Experience working with substance abuse, homelessness, families and groups
  • Excellent written and oral communication skills
  • Basic computer skills
