Job Description

Position Overview

The Housing Specialist assists clients in obtaining permanent housing and delivers services to clients consistent with the requirements of the funding contract and the policies and procedures of the agency. This person also assists in the oversight of the medical department of the facility.
  • Develop linkages with outside housing providers such as Real Estate companies, Public Housing Offices and Supportive Housing Providers.
  • Completes initial housing assessments and develops housing service plan.
  • Conducts individual and group counseling sessions focused on housing issues.
  • Develops and maintains housing resources and assists clients in finding suitable, affordable housing.
  • Assist clients in applying for various housing programs: HRA-1995, SRO’s, etc. Follow up on applications.
  • Develop and facilitate client workshops on skills and information necessary to obtain and maintain housing.
  • Conduct mock housing interviews to enhance client interviewing skills and escort clients to housing interviews, as necessary.
  • Meet weekly with clients to assist with the achievement of individual service plan goals and document housing progress.
  • Assist in the oversight of the medical department of facility
  • Document case management activities in accordance with agency and funding source guidelines and procedures.
  • Achieve monthly placement quota, as assigned.
  • Attend interdisciplinary team meetings, case conferences and housing meetings.
  • Keep supervisor informed of all relevant information.
  • Performs other related tasks as assigned.
  • Attend staff meetings and trainings to enhance skills that are related to job function.
  • Must be able to work flexible, alternate schedules.


  • High School Diploma, 3 years experience working with homeless population.
  • BA preferable. Direct experience in providing client services to homeless, mentally ill and/or chemically addicted population in a residential setting.
  • Bilingual a plus.
