Job Description

Position Overview


The Social Worker will work within the framework of a team to provide basic mental health and substance abuse counseling services including group work, individual work and referral as well as provide substance abuse education and psychoeducation to staff and participants. This individual will work to integrate Evidence Based Best Practices including MI and IDDT into all other services provided by the transitional residential program.
 
Responsibilities:
  • Recruit program participants from the transitional residences. Help the participants to develop the motivation to reduce substance use and to participate in recovery-oriented interventions
  • Assist in baseline screening and assessment of participants to assess health status, risk factors and lifestyle issues
  • Develop a relationship and working alliance with the program participants
  • Participate in the development of Individual Wellness plans for all participants
  • Identify and make appropriate referrals to community resources that provide substance treatment such as day treatment programs, detox, and rehab
  • Provide assistance to participants in the development of skills required in decision-making, problem solving and interpersonal skills
  • Assist participants acquire skills and supports for managing symptoms of both disorders and for pursuing goals
  • Assist clients to develop and use  strategies for maintaining abstinence and recovery
  • Responsible for recording accurately the interactions and or outcomes of all contacts between participants and partner agencies
  • Provide crisis intervention and advocacy services to all participants
  • Responsible for accurately recording progress notes, collateral contacts, and case management notes particularly regarding substance abuse and related issues


Qualifications


  • Master’s Degree in Social Work or Mental Health Counseling, license preferred 
  • Minimum of three years' work experience in providing participants with co-occurring disorders with case management and substance use services
  • Knowledge of Stage-Wise Interventions
  • Bilingual English and Spanish speaking preferred
  • Computer skills in word-processing, excel, and email required
