Position Overview

Services for the UnderServed (SUS) is a New York-based nonprofit organization that provides housing and support services to 25,000 individuals and their families living in New York City and Long Island.  SUS believes every New Yorker has the right to lead a life of purpose.

  • Provides case management services for a caseload of 20 clients.
  • Advocates for clients to obtain the full ranges of needed services and ensures coordination of these services.
  • Promotes linkage development and monitors the effectiveness of linkages with other service providers.
  • Ensures quality program services and referrals within program policies.
  • Ensures community follow-up to engage the client in care, promote compliance with medical appointments, and encourage client self-sufficiency and empowerment.
  • Assures timely intakes, assessments, service plans, and referrals.
  • Responsible for the assessment process, development of service plans and referrals.
  • Establishes and maintains case records.
  • Holds case conferences with all primary agencies that are involved with clients, obtaining case records when necessary.
  • Meets monthly with clients and more often as needed for counseling, follow-up, and advocacy.
  • Actively participates in internal and external training programs and committees.
  • Assists in distributing daily snacks.
  • Responsible for the planning and implementation of direct social services to tenants.
  • Conducts bio-psychosocial assessments. Makes community referrals when appropriate.
  • Participates in case conferences, staff meetings, trainings and seminars, client treatment planning meetings, and supervision.
  • Coordinates and maintains linkages with mental health and other appropriate service providers (e.g., support groups, psychotherapy) in the community.
  • Completes and files all progress and group notes, service plan goals, written assessments and other documentation in a timely fashion per quality assurance policy and procedures. Prepares monthly statistical reports.
  • Performs other duties as assigned by Director of Social Services/Clinical Supervisor and Program Director.


  • A Licensed Master Social Work (LMSW) with at least two (2) years’ experience working with HIV/AIDS, substance abuse, mental illness, and homelessness populations is required.
  • Knowledge of supportive housing, HIV/AIDS, and substance abuse is required.
  • Experience in assessments, service planning, and community referrals preferred
  • Excellent interpersonal and written and oral communication skills.
  • Ability to work as a member of an interdisciplinary team.
  • Bilingual English/Spanish preferred.
