Position Overview

Job Functions
  • Meet with Program Director, Director of Social Services, Psychiatrist, LPN Nurses, and Social Service staff to coordinate shelter residents to receive ongoing treatment in the community.
  • Establish substance use treatment goals and objectives with clients, as part of the interdisciplinary care planning process. Refer clients to outpatient and inpatient substance abuse treatment, as needed.
  • Maintain relationships with appropriate community resources to provide clients with treatment services. Identify and facilitate linkages with community providers
  • Facilitate weekly Substance Abuse groups including Relapse Prevention and Harm Reduction and other groups as needed
  • Complete Quarterly Substance Use Assessments
  • Conduct individual Substance Use Counseling
  • Provide crisis intervention services and if necessary, refer clients for psychiatric assessment
  • Coordinate joint services and treatment planning, assessment, reassessment and joint case conferencing with other service providers to clarify and define each provider's goals and objectives
  • Work with clients and case managers in developing treatment plan and identifying community based referrals for client to continue treatment upon move out
  • Attend and participate in regular staff training and scheduling meetings
  • Maintain confidentiality and privacy regarding all clients’ information
  • Complete incident reports in accordance with policy and procedures
  • Understand and follow the agency chain of communication
  • Ensure the safety and well-being of the clients on a daily basis
  • Report any actual or suspected abuse/neglect in accordance with policy and procedure
  • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
  • Maintain CPR/First Aid Certification
  • Assist in building evacuation during fire drills and building emergencies
  • A schedule that includes evenings and weekends will be required

  • Bachelor’s Degree in Social Work or related field required.
  • CASAC certification required
  • Preference is given to those who have experience working on a multi- disciplinary team and/or previous experience with homeless population
  • Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
  • Bilingual preferred.
